There are major changes late December in the way medications are exchanged at Butler Hospital. Here is an outline of the process
Medication Wraps
The exchange has remained the same as before. When a medication is used or expires, go to the hospital pharmacy and exchange the medication.
Drug Boxe Exchange
The orange drug box is no longer a 1-for-1 exchange and is now secured shut with a padlock and a green tag. A narcotic box is now secured inside the orange drug box.
Controlled Substance Exchange Process
If a controlled substance is used (Fentanyl, Morphine, Valium, Versed or Ketamine), you will complete a controlled substance form and exchange the black controlled substance box at the window at Butler Hospital. You will return to Base 1 and follow the locking procedure below.
Orange Drug Box Locking Procedure:
Before locking the orange box, complete an inventory sheet to ensure nothing is missing. You are accountable for the medications in the box. Completing an inventory sheet ensures that medications are not missing and completes a check-and-balance for the orange medication box. Another option is using the short form; however, you are assuring that nothing is missing from the drug box and are not demonstrating that you completed a drug box inventory. You are accountable for missing medications.
Green Tag
In order to receive a green tag from the supervisor, you will need a completed form with the following information:
Why the orange box was opened
Drug Box Number
Narcotic Box Number
Narcotic Box Red Tag Number
Narcotic Box Expiration Date
After receiving the green tag:
Make sure the black narcotic box is in the drug box
Lock the orange drug box with the padlock
Place the green tag on the drug box
Long Drug Box Inventory (Ensures that the drug box was checked)
Short Drug Box Exchange (Did you really check your box?)
