Truck Check

Daily Truck Checks will be completed digitally starting April 23rd.  Truck Checks are to be completed at the beginning of every shift.   When you complete your digital truck check, it is sent directly to the supervisor for review.  The QR Code can be found inside the refusal box (Sticker) or by scanning the QR Code Below.  You can complete the form below or click the link as an alternative.

Scan the QR Code or click the link below.  Complete your truck check and select submit.   The truck check should only take around 5-10 minutes to complete.  

NOTE:  The form below does not work well with a cell phone!!!
